Vismodegib is a small molecule that selectively binds to and inhibits Smoothened (SMO), a transmembrane protein essential for Hedgehog (Hh) pathway signal transduction. In normal tissue, the Hh pathway is largely inactive in adults. However, in >90% of BCCs, loss-of-function mutations in PTCH1 or gain-of-function mutations in SMO cause constitutive pathway activation, driving tumor cell proliferation and survival. By inhibiting SMO, vismodegib blocks downstream transcription factor GLI activation, resulting in growth arrest and apoptosis of BCC tumor cells.
