Issued by a UAE-licensed physician, on a recognised prescription pad, dated within a recent window, and clearly specifying the drug name, strength, dose and duration.
A brief report from your treating doctor (oncologist/specialist) explaining the diagnosis, treatment plan and required duration of therapy. One page is usually enough.
Emirates ID or passport of the patient. For a family member ordering on the patient's behalf, both IDs are useful.
MOHAP's personal-import framework is generally designed for a reasonable personal-use supply — typically enough for one treatment cycle up to roughly three months, matching what the prescription specifies.
