What is Ocrelizumab? Understanding the Active Ingredient
Ocrelizumab (INN) is a humanized anti-CD20 monoclonal antibody developed by Roche/Genentech. It selectively targets and depletes CD20-positive B lymphocytes — a key driver of inflammation and neurodegeneration in multiple sclerosis. Unlike Rituximab (a chimeric antibody), Ocrelizumab's humanized design results in significantly fewer infusion-related reactions and improved tolerability.
Ocrevus was approved by the FDA in March 2017 as the first and only disease-modifying therapy for PPMS, based on the landmark ORATORIO trial. It also demonstrated superiority over interferon beta-1a (Rebif) in two Phase III trials (OPERA I and OPERA II) for relapsing MS, reducing annualized relapse rates by 46-47%.
The convenience of twice-yearly dosing (every 6 months after initial loading) versus daily/weekly alternatives has made Ocrevus one of the most prescribed MS therapies globally, with over 300,000 patients treated worldwide since approval.
